Under the direction of the Aging and Disabilities Services Program Manager, this position supports older adults (55 years+) and other adults with disabilities to assess and coordinate needed services to enhance their quality of life and support them to maintain stable housing and live independently. This work is primarily accomplished in concert with individual residents by assisting residents to connect to programs and community resources that enhance their physical, social and mental well-being.

SCOPE OF WORK:

This work is primarily accomplished in concert with individual residents by assisting residents to connect to programs and community resources that enhance their physical, social and mental well-being.

REQUIRED FUNCTIONS AND RESPONSIBILITIES:

The following duties are illustrative only (i.e. not all inclusive)

Required Workplace Standards:

1. Works regular and reliable hours on and off site as required;

2. Attendance must be punctual and consistent;

Essential Functions:

1. Provide general assistance and advocacy related to supportive and social services to all residents by observing and working with residents to establish trust and encourage appropriate use of services and local resources to maintain a high quality, independent life.

2. Meets with residents to assess, counsel, recommend or resolve resident issues impacting their personal life, health and well-being, social connectivity and risks to their housing. Meetings can occur in a resident’s home or at an off-site location as necessary;

3. Identifies service barriers and unmet needs and develops Individual Service Plans to meet goals;

4. Provides referrals to local partners/resources and other appropriate supportive services to assist residents with meeting their needs and goals. Monitors progress and provides support;

5. Help residents apply for entitlement programs and other social service supports. Works with residents to establish trust and encourage appropriate use of services to maintain their independent life style;

Prepares and maintains appropriate case file records, collects resident data, prepares written and oral progress reports as requested and attends to other administrative duties including departmental meetings and serving on agency committees;

7. Work cooperatively with housing management, residents and their families or other informal supports to resolve tenant issues that jeopardize stable housing or provide information on alternate housing;

8. Act as liaison with local care providers and hospitals to ensure successful discharge from care facilities and transition back to home;

9. Works with other staff members and residents to establish a sense of community and promote a positive social climate that fosters residents’ psychosocial well-being and supports an inclusive and respectful environment for everyone;

10. Coordinates community building activities and provides educational and socialization opportunities that support decreased isolation and increased quality of life;

11. Recruit and train volunteers and oversee their activities. This may include oversight and direction of student interns or AmeriCorps members;

12. Participates in local networks or councils and engage in professional development training on topics such as; Fair Housing, Aging, Disabilities & Elder Care, Behavioral Health, Co-Occurring Disorders, Bed Bugs, Hoarding, and Health & Wellness;

13. Work with resident councils to meet program objectives;

14. Actively participate to support the EHA Bed Bug Protocol, including, but not limited to, provide residents educational resources about bed bugs, participate in Bed Bug Briefing for residents who have infestations, and meet with residents who have infestations and create a day of treatment plan.

MINIMUM REQUIREMENTS TO PERFORM ESSENTIAL FUNCTIONS

The Service Coordinator position requires an ability to work in a complex and non-traditional human service setting with a diverse population. A combination of education and experience that results in knowledge of the human service system and experience working with persons with disabilities and the elderly is essential.

REQUIRED EDUCATION AND EXPERIENCE:

Option #1:

A Bachelor’s degree in social services, health or human services;

PLUS

Two years of paid direct service experience with senior and adult populations with mental health and co-occurring health issues;

OR:

Option #2:

An Associate’s degree in social services, health or human services;

PLUS

Four years of paid direct service experience with senior and adult populations with mental health and co-occurring health issues;

OR:

Option #3:

Six years of paid direct service experience with senior and adult populations with mental health and co-occurring health issues

REQUIRED KNOWLEDGE OF/SKILL IN:

§ In-depth knowledge of available social services related to Seniors and/or adults with disabilities and an understanding of eligibility requirements to make appropriate referrals;

§ The ability to build trust and connect with residents using professional boundaries and good judgement;

§ Excellent oral and writing skills.

REQUIRED ABILITY TO:

§ Relate to residents in a professional, caring manner;

§ Work independently and manage time and tasks efficiently in a non-structured setting with minimal supervision;

§ Work professionally with co-workers, residents, social service agencies and the public;

§ Comply with safety procedures pertinent to the work environment to ensure personal safety and safety of residents;

§ Prepare and write reports at a professional level;

§ Speak and understand English and follow oral and written directions;

§ Keyboard equivalent to 35 WPM.
